Community Care Services

Monash Community Care provides a comprehensive range of home and community based services and activities to the assist frail aged people and people with a disability, and their carers.

These services are known as Home and Community Care (HACC) services and are provided to assist eligible residents of the City of Monash to remain living in their homes and in the community, and importantly aim to maintain their well-being, independence, and dignity.

What is Home and Community Care (HACC)?

The HACC program is funded jointly by Commonwealth, State and Local Governments. The aim of HACC services is to provide basic support and assistance to eligible people living at home to assist them maintain their independence and their capacity to remain living within their community.

There are a range of HACC services available through the Monash Community Care:

  • Assessment & Referral
  • Domestic Assistance
  • Respite Care
  • Home Maintenance
  • Community Transport
  • Personal Care
  • Food Services - Meals on Wheels
  • Planned Activity Groups (PAG)
  • Shopping
  • Home From Hospital

Monash Community Care services are provided by skilled and experienced Council staff dedicated to assisting people to maintain their quality of life and general well being.

Staff provide a professional, personal and caring service. They are skilled in carrying out specific tasks and have considerable training in issues concerning the frail aged, people with a disability, and their carers. Staff are required to undergo police and reference checks.

All staff are required to adhere to legislation relating to matters such as Occupational Health and Safety (OH&S) and Privacy.

Assistance can be provided on either a short or long-term basis depending on clients' needs. The level of support required is determined jointly at the time of the initial assessment. The services provided seek to be responsive to clients' needs, taking into account their individual situation.
