Guidelines and Nomination Form
The Monash Disability Awards recognise achievement by organisations, groups, businesses and individuals who have contributed to improving the life of people with a disability.
About the Awards
The Monash Disability Awards were launched in August 2008. The Awards are in line with Council's Disability Access and Inclusion Framework and Action Plan 2009-2013.

The awards will be presented on International Day of People with a Disability (3 December) or at another agreed time, at a ceremony arranged by Council.
Award winners will be notified prior to the award presentation event. The award winners will be publicised through Council's website and the local media.
Award Categories
1. Access
This category refers to physically accessible premises and/or outdoor environments. Organisations such as retail stores, restaurants, recreational facilities, schools, sporting clubs, churches that have made significant progress in making living or working places, open spaces or other designated space accessible could nominate or be nominated.
2. Service delivery
Organisations, agencies or individuals who have developed programs, services or processes that have made a significant contribution to improving the lives of people with a disability can nominate or be nominated.
- training and employment programs that enhance employment or career development opportunities for people with a range of disabilities
- programs that ensure access to and participation in leisure and recreation services and programs
- development of structures that encourage the participation of people with a disability in decision making processes, access to information, access to the internet, access to funding etc.
3. Innovation
Nominations can be received by individuals and/or organisations and agencies who have developed innovative and inclusive programs or other initiatives that have significantly improved the lives of people with a disability and/or their carers and families.
- for example young people with a disability, carers or people from culturally and linguistically backgrounds
- an initiative that promotes community awareness and aims to change attitudes towards people with a disability
4. Business
A business, which has significantly contributed to improving the lives of people with a disability. Examples could include but are not limited to:
- modification of premises to make these accessible and/or inclusive
- development of recruitment programs or opportunities for the employment of people with a disability
- advocacy programs promoting participation and inclusion of people with a disability
Who can nominate?
These awards are open to anyone who lives, studies, has property and/or works in Monash and are involved with or contributing to the disability area (i.e. businesses, community, groups, organisations and individuals).
How to nominate?
Information supporting the nomination should also be provided with the nomination form. If the space provided is not sufficient please attach additional information.
The information should be provided in a clear and concise manner, focusing on positive attributes, achievements and outcomes and highlighting the overall strengths of the initiative and or contribution.
The nomination should address the selection criteria.
Selection criteria
Nominations will be assessed against the following criteria:- The degree to which the achievement has enhanced the lives of people with a disability, their carers and/or families
- The degree to which the achievement has contributed to physically accessible premises
- The significance and/or impact of the achievement to the development of the disability sector
- Is the initiative new and/or existing, is it disability friendly and/or inclusive of people with a range of disabilities
- The degree to which the achievement has included people with a disability in the development and decision making associated with the initiative
- The level to which the initiative or achievement involved partnerships with other organisations or individuals
- The degree to which the achievement has been maintained
- The level to which the achievement has contributed to the employment of people with a disability
