School Entry Immunisation Certificates

A School Entry Immunisation Certificate is required when your child starts school.

 If all vaccinations have been given in Australia:
School Entry Immunisation Certificates are sent automatically from ACIR or can be requested by phoning ACIR on 1800 653 809. They are also available upon request from Medicare branches.

If vaccinations were given overseas contact Monash Immunisation Services on 9518 3540 or visit either of the Council officer.

 If all vaccinations have been given overseas:
  • Visit either of the Council offices with all immunisation records.
  • If the child is up to date to 4 years a School Entry Immunisation Certificate will be issued at that time.
  • If the child is not up to date and requires further vaccination advice will be given on how to complete the required schedule prior to obtaining a certificate.

Vaccination Information

Human Papilloma Virus Vaccine

The Human Papilloma Virus Vaccine (HPV) Gardasil® is to protect girls against 70% - 80% cervical cancer.

Final trials of the Gardasil® vaccine found it to be nearly 100% effective against HPV, a sexually transmitted disease, which causes approximately 70 - 80% of cervical cancers.

Since the introduction of routine Pap tests and a national screening program, the number of deaths from cervical cancer annually has more than halved. Unlike other cancers, a HPV virus spreads cervical cancer through genital skin contact. Currently four out of five women will have HPV at some time in their lives.

The vaccine will be made available to girls aged 12 - 18 years as part of the school immunisation program conducted by Monash City Council. It will be initially offered to girls in Years 7, 10, 11 & 12 in 2007. Students in Years 8 & 9 in 2007 will be offered the vaccine in 2008. The vaccine will then become part of the routine school immunisation schedule and will be offered to students in Year 7 annually.

Gardasil® vaccine is given in three doses at 0, 1-2 and 4-6 month intervals.

 The vaccine will also be free for women aged between 18 - 26 years from July this year and is available through their GP.

Meningococcal Disease
Meningococcal C is available free to children aged 12 months and those born on or after 1 January 2002. Older children can receive the vaccination for a fee of $50.00 (refer to attached form).

Chicken Pox
Chicken Pox Vaccine is free to all children when they turn 18 months of age and non immune children in Year 7 at Secondary School. For all other children, the Chicken Pox Vaccine is available at all immunisation sessions for $50 per vaccination. This is to be paid for prior to the session. Forms are available from the Oakleigh office and the Monash Immunisation Services office at the Civic Centre.

Hepatitis B
Hepatitis B Vaccine is available at immunisation sessions at a cost of $15.00 per adult dose or $45.00 for a full adult course. This to be paid for prior to presenting at a session.
