Christmas Morning Melody Day Trips Ballot Form
Instructions (please only send Ballots if you can commit to a trip)- If you were on the previous trip your ballot will be put aside.
- If you were on the 2007 Christmas Morning Melodies Day Trip you ballot will be put aside.
- You will find out early if you are going on a trip to allow plenty of time to plan for the trip and to confirm your attendance.
- Ballots are randomly selected until we have filled the trip, there are 51 places available.
- If successful and you OR your nominated guest can NO LONGER attend your place will be given to someone on the waiting list— you can NOT choose another guest.
NOTE: If you are selected for a trip and can no longer attend, you will be classified as having participated which may affect your chances for the next trip.
Cancellation Fees: A minimum 7 working days notice of cancellation is required. Cancellation within the 7 day working days notice will require a medical certificate to avoid being invoiced the full recovery cost. Full recovery coast for the Christmas Morning Melodies Day Trip will be $37.00. If you book in for a guest and they can no longer attend they will be invoiced for their full recovery cost. Your guest will need to supply a medical certificate.
